Plant phytosterols 2 g (sterols / stanols)

The European Union rarely allows a precise figure. Here it does: 7 to 11 % less bad cholesterol (LDL) in three weeks.

Evidence level : solideGUIVONIZ index : 87Dose : 1.5 to 3 g a day, with meals

The short version

What it does They block part of the cholesterol your gut absorbs. At 2 g a day with meals, LDL falls by 7 to 11 % in two to three weeks — it is measured, and the European Union permits saying so.

What it does not do They do not replace a statin if your doctor has prescribed one, and they do nothing for triglycerides. And they are useless if your lipid panel is already good.

What the studies say

Ras’s meta-analysis pooled 124 studies at an average dose of 2.1 g a day: an LDL fall of 6 to 12 % depending on dose, and the more you take the more it falls — about 7.6 % between 1.5 and 2 g, 10.3 % between 2.5 and 3 g. On that basis EFSA authorised a quantified health claim in the European Union, which exists for almost no other supplement.

This is an effect on a marker, LDL, not on counted heart attacks. No large trial has measured cardiovascular events on phytosterols. And the matrix matters: incorporated into a fat or a dairy product, they work better than in a juice or a chocolate.

If your LDL is high and your doctor gives you three months to act through diet, this is the best documented lever alongside psyllium. The two add up, through different mechanisms.

How to take it

Relevant with high LDL, a family history, or alongside a dietary approach agreed with your doctor. Pointless if your lipid panel is normal — this is not an all-purpose prevention product.

1.5 to 3 g a day, spread across main meals. Taking it with a meal is not a comfort detail: in the meta-analysis, a single morning dose on an empty stomach produced no significant fall. Combine with psyllium if you want to stack them: the two act at different steps.

Lipid panel before, then after two to three months — the maximal effect is reached in two to three weeks, but the blood test interval is practical. If LDL has not moved, first check you are really taking the dose with meals.

What is inside

Plant-derived sterols or stanols, often esterified to be fat-soluble. Stanols are the hydrogenated form of sterols; both lower LDL, and with stanols the fall tracks the dose a little more evenly.

Read the dose as FREE sterols or stanols, not the ester weight: a stanol ester weighs nearly twice the active molecule. An esterified form in a softgel absorbs with the meal better than a dry powder.

Doses below 1.5 g a day, which do not reproduce the studied effect. Not advised during pregnancy and breastfeeding, or in children under five. Phytosterols also slightly reduce carotenoid absorption: keep colourful vegetables on the menu.

Cautions

  • Pregnancy and breastfeeding
  • Children under five
  • Ongoing lipid-lowering treatment: discuss with your doctor

The three options

Named beta-sitosterol, 800 mg

Watch out for red yeast rice

Several highly ranked “phytosterols” in this aisle actually contain it: its monacolin K is chemically identical to a statin, does not have the same risk profile, and should never be taken without medical advice. This one is clean: beta-sitosterol named, 800 mg per dose, over 2,000 reviews. Read the list before buying.

For whom — High LDL followed by a doctor, prolonged use.

2 g a day in mealtime capsules

WARNING: contains a natural statin

2.9 mg of red yeast rice monacolins on top of the phytosterols. Monacolin K is the same molecule as lovastatin, a medicine. Europe capped its dose in 2022 and the French food safety agency advises against combining it with a prescribed statin. We list it so you can recognise it, not to recommend it.

For whom — The default choice, alongside a lipid panel.

Enriched margarine or yoghurt

No link: it is in the chilled aisle

Sterol-enriched margarines and yoghurts are not sold by post. On the packaging, look for the 0.8 g per portion and the authorised European claim.

For whom — Anyone who would rather have the food than the supplement — and it is the matrix where the effect is best documented.
